Position: Care Coordinator

Position Duties: The Care Coordinator is responsible for ensuring the provision of direct services and service coordination to qualified individuals. This includes developing and updating Plans of Care, monitoring services in the plan, doing regular visits with individual, providers, and other direct supports. The Care Coordinator is expected to meet the requirements established by Senior and Disability Services (SDS) as outlined in the SDS Care Coordinator guidelines of the Provider Packet.
Minimum Qualifications: Education and Experience: Bachelor's degree in social work, human services, psychology, or other related field with one year direct care experience or three years direct care experience. Prior study in or work experience with developmental disabilities is preferred.
Communication Skills: Ability to effectively communicate with consumers, families, other staff members, personnel of other agencies, and the general public. Ability to write in a clear, accurate, and grammatically correct manner.
Equipment: Must have reliable vehicle. Ability to use personal computer, fax machine, and other appropriate office equipment. Microsoft Word and Excel proficient.
Certifications and Clearances: Must have current care coordination certification or obtain within the first 90 days of employment. Must have a valid Alaska Driver’s license, required level of automobile insurance, background check and TB test. Must obtain CPR, First Aid certification. Must obtain professional care coordination certification within six months. Other training may be required.
Knowledge: Demonstrated thorough knowledge of HCB waivers, community social service agencies and programs. Demonstrated knowledge of behavior management programming and techniques, systematic skills training, and assessment techniques. Demonstrated understanding and ability to apply principles of normalization, least restrictive approach, and service planning. Ability to exercise patience, understanding, and concern for each consumer’s health, growth, safety, and enjoyment of life.

Must obtain CPR, First Aid, and Nonviolent Crisis Intervention certification. (If not certified, must certify within 30 days of employment.) Other training may be required.
